Evercare coordinates care for thousands of patients in over 1100 nursing homes across the country. Evercare has contracted relationships with many nursing homes and can provide care management for patients in those homes. Evercare works with select Medicare Advantage plans and is contracted with the UnitedHealthcare Nursing Home health plan.

How to get the Evercare Nurse Practitioner Care Model:

Patients are eligible for the Evercare Nurse Practitioner Care Model if they reside in a nursing home that offers Evercare Services.

AND

Have coverage through the UnitedHealthcare Nursing Home health plan.
